[GENERAL PROVISIONS]

§ 1

ZIĘBA BŁOTNY ADWOKACI sp. p. website processes personal data provided in the contact form and in cookies. przetwarza na stronie internetowej dane osobowe zawarte w formularzu kontaktowym oraz w plikach cookies.

§ 2

The personal data Controller is the partnership ZIĘBA BŁOTNY ADWOKACI sp. p. with its business seat at ul. Gwiaździsta 10 lok. 5 in Wrocław (53-413), KRS: 8992895612, NIP: 0000888736, REGON: 388433296, e-mail: kancelaria@zieba-blotny.com, ph. 788 964 758 (further referred to as „the Controller”).

§ 3

Personal data is processed in accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (further referred to as RODO), including article 6 (1) (a) of RODO.

§ 4

Providing personal data is voluntary, yet indispensable for the use of the website and contact form.

§ 5

Subject to rules provided in RODO, the User shall have the right to access the personal data, to request to rectify, supplement or remove the data, to limit the processing of the data and to object to the processing, to transfer it, to withdraw the consent for processing and to file complaint with the President of the Personal Data Protection Office.

§ 6

Personal data shall be made available to the following recipients only: entities authorized by the Controller to process the data and entities processing the personal data on behalf of the Controller under contracts concluded with the Controller, including the owner of the server, holding the website and to the IT staff members.

§ 7

Personal data shall not be transferred to a third country within the meaning of RODO, nor shall it be used for the purposes of decision-making process consisting in automated processing, including profiling.

In case of consented personal data processing, the User shall have the right to withdraw the consent at any moment. The withdrawal shall have no effect on legality of the consented processing executed before the withdrawal.

The consent to the personal data processing is voluntary, yet indispensable in order for the request presented to be answered. Failure to provide the personal data shall make the use of the contact form impossible.

The personal data provided in the form shall be retained as long as it is required for the actions undertaken upon the request.

Cookies are processed until they are removed by the User or until deadline set out up front in the cookie’s parameters.

The Controller uses services of Google Analytics in order to receive on the basis of cookies statistical information about users of the website and their actions thereon. Data on the users and their activities are send to Google Inc. with its seat in United States of North America in a way preventing identification of the individual (anonymized).
